iThrive's 21-Day Gut Reset: Terms & Conditions

I acknowledge that the nature, purpose, and details of iThrive’s 21-Day DIY Gut Reset Program have been explained to me.

I understand that this program solely focuses on Gut Reset and aims to address issues like bloating, gastric, and digestive issues and does not encompass treatment of any/ all other pre-existing diseases or health conditions including leaky gut which require separate intervention.

I do hereby declare that I am fully aware that services relating to Blood tests, Root Cause Analysis, Supplements as well as guidance and consultation are not a part of this program. I do hereby declare and agree that in case of requests for such additional services from my side, I can get in touch with the iThrive team to provide such services at additional costs as per iThrive's policies. The decision of iThrive shall be final in these matters.

I agree and understand that iThrive does not handle acute conditions under this program. I may need a Primary Care Physician / Specialist / Family doctor to attend to any medical emergencies during my 21-day Gut Reset Program.

While experts have designed this program to help address and resolve gut issues, I understand that this program does not expressly guarantee any reversal of Gut issues completely, and based on my adherence to guidelines and instructions to the program may help me alleviate my gut health symptoms like digestion, gastric issues, bloating, etc.

I acknowledge that I will receive various materials as part of the program, including but not limited to a habit tracker, meditation guides, care card, and a recipe booklet. I agree that I will not duplicate any of the materials shared with me through this program in any form, including copying, scanning, photographing, or any other method.

I also agree that I will not share any of these documents or materials with anyone outside of the program, including sharing via physical copies, electronic means, or any other form of communication. I understand that if I violate any of the above terms, iThrive reserves the right to take legal action against me, which may include filing a case for breach of agreement and confidentiality. By participating in this program and accepting these materials, I hereby agree to the above terms and conditions.

I hereby release and discharge Thrive Tribe Wellness Solutions Pvt Ltd from any potential claim, loss, damage, etc. (“Loss”) that I may suffer or have against iThrive, as a result of the 21-day DIY Gut Reset by iThrive, if such Loss is due to - (i) my non-adherence of the instructions and guidelines recommended by iThrive, or (ii) concealment, whether complete or partial, of any medical condition, history, treatment or allergy(ies) that I may have or I am suffering with.

I understand the key to my Gut Reset is my commitment to comply with prescribed treatment recommendations. If I don’t follow the program with reasonable consistency, my progress will likely be stalled.

I hereby agree that I will not make any negative comments or reviews in public forums, social media, press releases, or any other medium about iThrive or its products, services, or business practices. I would instead pursue amicable communications to help address and resolve any issues, if any, related to the program. All further claims/disputes shall be subject to the exclusive jurisdiction of courts in Pune.

Authorization:

I agree and hereby authorize and give consent to iThrive to access, use, and share my details mentioned in the form including personal details as well as my health history, diagnostic test results, prescriptions, medications, and medical records, with the iThrive team. I agree and give consent for the same to be used for research purposes.

I understand that I have the right to revoke this authorization at any time by providing an advance written notice to iThrive. However, any actions taken based on this authorization before the revocation will remain valid.

I acknowledge that iThrive has explained to me my rights and how my health information may be used and disclosed. I have had the opportunity to review and ask questions.

I do hereby declare and understand that once iThrive's 21 Days Gut Reset Program has been booked and payment has been made by me, it cannot be canceled for any reason whatsoever. As per the Company Policy, iThrive does not offer partial or complete refunds, substitutions, or cancellations for any reason whatsoever. I hereby declare and undertake that I have thoroughly read the entire content and all the terms and conditions herein including the refund policy and shall make no claims in this regard.
